Requests will be processed the business day in which they are received. A minimum advance of $1,000.00 is needed to access your line of credit.

By sending this advance request form, I hereby authorize you to grant an advance based on the terms of my LoanLinerŪ Home Equity Plan Credit Agreement and Truth-in-Lending Disclosure. I understand that my repayment terms will be determined as follows:

Up to $10,000.00	72 monthly payments
$10,000.01 -- $25,000.00	96 monthly payments
$25,000.01 -- And above	120 monthly payments

We will send you a notice indicating your new payment amount and when your next payment will be due.

If your loan is currently being paid through Payroll Deduction or Direct Deposit, we will contact you to make any needed adjustments.

New advances are available on the three Prime Rate Indexes and the Treasury Bill Index. The maximim rate imposed on Home Equity line of credit loans is 15.00%. The minimum rate imposed on Home Equity line of credit loans is prime plus zero. Please refer to our Rate Page for the current rate.
